Answer to Question 1
Daily journal
As they are read by the service users on a regular basis it must be kept free of unnecessary jargon
Everything must be unbiased reports on what has been happening, not personal attacks about people and events
It must be created in association with the service users
If it contains any confidential information then it must comply with the data protection act

Contact details
Must comply with the data protection act
Must be checked and kept up to date
Only staff who need to have access to the information for important work reasons should be able to access it
A backup should be kept, but still without breaking any of the above guidelines

Incident reports
Must be detailed and precise. "A service user cut himself on a chair in one of the rooms today" is totally useless, it would need to include:
Which service user?
Which chair?
What room?
Did he cut himself on something else while on the green chair or was it on the chair itself?
Was there anything wrong with the chair before hand?
Has the chair been damaged by the incident?
Was it a small scratch or a serious cut?
Was any medical assistance needed?
Was anybody else involved?
Was it an accident?
Could anything have been done to prevent it?
Did anybody else see what happened?
